Yes, it is possible with pd...

But I think this would be useful if you need something like a list of
words to work with the labels in realtime or any other especial
application.

There are good pieces of software to do this task for analysis. The
most famous is Praat - http://www.fon.hum.uva.nl/praat/

well more like audiofiles that can be tagged
hopefully the notes will be stored in a db and then yes you could make a
karaoke or a sound map.

I am reading of aubio library too. wonder if I can save the associated
notes as separate text files, xml files maybe.

WAV files can store notes, IIRC.  Or at least Audacity can tie text  
to audio projects.  I am sure other audio editors can too, but I am  
not sure there is currently a way to get that text in Pd.

Hello,
  even better, the associated notes could be saved inside the soundfile 
as well like it is done with mp3 tags.
  d_soundfile.c would have to be modified for reading/writing those 
comments, anyone knows how?
