ERROR:  cannot read block 2 of pg_toast_17939_idx: Input/output error" 
 
what does this  mean?.
 
After some investigation  found out that this happened after a vacuum of a table
vacuum t;ERROR:  cannot read block 95 of t: Input/output error
 
Somebody please help to fix this problem

I saw this error 
PostgreSQL server: FATAL 1: Sorry, too many clients already in /usr/share/php/phplib/db_pgsql.inc on line 49.
 
It seems like I have to increase the maximum connections in the postgresql.conf .Is that the right step to fix the problem..Also I am seeing lot of idle processess
