The configuration file (php.ini) should have been placed in the system folder (C:\WINNT). Instead, InstallShield placed it in the folder C:\Documents and Settings\Administrator\WINDOWS. This was problematic as the executable could not find the ini file to read it. Since it was IIS, I could not run phpInfo() to detect this, instead I was presented by the "The PHP CGI cannot be accessed directly." error since the default value is set to 1.
