Yes.

To just generate a PDF on the fly from FORM (or database) data:
http://php.net/pdf

To create a PDF that actually *is* a FORM of sorts:
http://php.net/fdf

I'd recommend trying the first one out and see if that will work,
before diving into the more complicated FDF stuff.
