1) Did you really do "$PATH/qmail start"? That doesn't make sense.

2) "qmail start" starts qmail. It's not a part of the installation
   tests.

3) "env" and "multilog" don't appear in the same script in the LWQ
   configuration. "env" is only used in "qmail" and
   /var/qmail/rc. "multilog" is only used in
   supervise/qmail-send/log/run and supervise/qmail-smtpd/log/run.

I think you need to go back through LWQ and verify the contents of
your startup scripts.

Posting the output of "sh -x /usr/local/sbin/qmail start" might help,
too.
