[QUAD-L] inspire surgery for sleep apnea

hi team

 my sleep apnea has been getting worse and being a quadrature doesn't help. 
Dental devices  and CPAP is no longer effective-- for example I can't keep the 
CPAP mask on when I have bad night spasms and dysreflexia

 after a long battle my insurer  is approve me for surgery this July 30 to  
install the Inspire system. Which is a pacemaker type device that is surgically 
implanted to send electrical signals  to the back of the throat that basically 
bypasses interrupted breathing. The recovery and training the system will take 
months.

 I'm wondering if anybody else has gone through this yet?  if you know how our 
quad bodies respond? Apparently it's pretty common in Europe but it's just 
taking off here in the states.

in my younger years as a quad I did fine on rojo  and Jay cushions – – 
pneumatic and gel respectively.

 At 30 years post my body had had enough and I developed a sore that kept me in 
bed for a year

 I tried more than 20 different products on the market

 the only cushion that worked dependably for long hours of sitting in with no 
breakdown were the Ride Cushions.   highly customized and highly durable and

there is a cushion made that does have hole in middle but i think it is custom 
made and/or you must have company measure & build it just for you.
there is no reason that so many wheelchair cushion companies cannot make a 
cushion that pressure maps good and prevents pressure sores. no excuses.

I am going through process of getting new electric wheelchair and trying to get

I have invacare storm series but invacare went bankrupt, and dealer wants me to 
switch to Quantum R-TRAK rear wheel drive.Does anybody with ezlock have one for

I heard that ezlock is making one for R-TRAK  but I do not see it on their list

share their experience and would appreciate some assistance!

about the expense.

By the way, I'm also 56, C5/6,  and will be a quad for four years this year (a 
surfing accident in 1984). I know all of us on here are black swans who have 
survived the odds for a long time. But it was extra cool to hear about two of 
you who have had such a similar journey…

I'm 56, (C5/6 for 40 yrs) and broke my femur --just above the knee-- in a 
car accident 2 yrs ago.  Even with my advanced osteoporosis it healed well with 
no bracing needed.  I also had a burst fracture at L4 that was a whole 
different story.  We thought my leg was causing autonomic dysreflexia (AD) but 
I insisted it wasn't because the AD changed depending on the position of my 
back.  I pressed my docs enough for back x-rays that once a broken vertebra was 
found (1.5 months after the accident) they decided to do a full spine CT.  
Vertebrae were too fragile for plates & screws so I had a cement injected into 
the cracks to stabilize the vertebra and it helped A LOT.  (Did this on a 
sacral vertebra in '08 after a  bad transfer caused a nerve pinch.)  I still 
had slight AD & sweating after the lumbar procedure so I'm still getting 
epidural cortisone & lidocaine injections at a spine clinic every 3 months to 
control the remaining AD symptoms.

I had my first Ileo-conduit in 1986 from bladder cancer. Then In 2014 I kept 
having UTIs all the time. The kidney doctor didn’t know how to do the surgery 
that was needed to make another ileo-conduit. She wanted to put me on hospice. 
We started calling every kidney doctor and that was able to do the surgery. he 
got me on the table within two weeks. we had to have a long talk and explain to

Sorry, I just saw this email from months ago. Have you made a decision on what 
to do?

My late best friend had her ileostomy after years of incontinence, occasional 
impactions,  and discomfort with every bowel movement. She hated the idea of 
the colostomy bag, so she resisted her doctor's advice for years.

When she finally just had to have the procedure, she was really astonished. The 
care of the bag was an annoyance, and she was very insecure about its impact on 
her sex life (fortunately, on this latter issue, her FWB was very accepting of 
it),  But otherwise, it was such a relief she wished she had done it years 
earlier.  She had much less pain, much less disruption, and regained all of 
those hours she lost to bowel care.

Until I found the peristeen bowel irrigation system, my next option was going 
to be an ileostomy. My friend's experience greatly reduced my resistance to the 
procedure.  if that's what's needed someday, I've made my peace with it.

Being able to intercom hands-free to my attendant, no matter where in the house

Also, when my wife and kids are not around, I know my wife appreciates the fact

You get a couple of the cheapest Amazon Echos, assuming you have Internet, and

You get a couple of the cheapest Amazon Echos, assuming you have Internet, and 
turn the volume up because that's both the speaker and the microphone, and keep 
it nearby. Should work fine

I've never used functional electrical stimulation that way. However, when I was 
using the FES REGIS bicycle to get a lower limb/cardio workout, I noticed an 
overall decrease in core spasticity. It could have been from stretching muscle 
groups that often trigger,  or it could have been the electrical current 
interfering with spastic releases. In either case,  while I was on that system, 
I had very low spasticity.

  I'd say, give it a try. TENS  is a very mainstream technology now and 
shouldn't cause any harm to try.

I have never used a TENS unit for spasms nor have I ever heard of anyone using 
TENS for spasms.  Spasms are usually handled systematically using medication 
like Baclofen.  That said,there is no risk  in trying it.  If it truly is 
localized to the abdomen maybe Botox or acupuncture

Thanks Don, all of us need support now and then. We appreciate your offer.

A lot of you probably already saw this article in new mobility. I think it's 
worth posting the link here because it focuses exclusively on long-term SCI.  
the article sounded, at least to me, as a bunch of wise quads and para sharing 
the benefits of their experience with people more recent to their disability.   
I would've liked to hear more advice specifically geared towards managing 
long-term disability, along the lines of many conversations on quad list. 
still, it was reaffirming for the magazine to simply gather people with 
long-term SCI-- a population that often goes overlooked or worse, is assumed 
not to exist at all.

It's been very quiet in this group. I hope everyone is doing well. I think that 
now that the pandemic has passed things are getting back to normal, mostly.

The one silver-lining i found was that it forced us to embrace technology to 
maintain our social connections. I had never even heard of Zoom, and suddenly i 
was scheduling meetings and support groups using this new tool.

To that point, if anyone on here is ever feeling isolated please know there are 
online support groups you can join to get that interaction that is so 
important. I host a Men's Disability group via zoom every Thursday at 4pm AZ 
time. I'm sure there are other groups out there as well.

[QUAD-L] sharing game changing products

In a recent post I mentioned some of the products that have been essential to  
successfully living my life as a quad (C5 –6, 38 years post surfing injury). I 
wanted to expand upon and share my list with everybody. I think it would be 
really interesting to hear everybody else's list as well.

Products that change my life:

Sure Hands Lift – – I'm still using the lift I bought 20 some years ago when 
doing board and pivot transfers became too rough on me and my attendants.  it 
has never broken down and only needed a replacement battery once.  nothing gets 
me in and out of bed faster-- no Hoyer type sling. It also places me in exactly 
the right spot in my wheelchair.  I only wish they had a travel version.

Peristeen Bowel management program--  there is more about this my last post, 
but basically it is made my   bowel program faster, more dependable, easier on 
my body, and easier on my attendants.

 Dragon dictation system-- I use this every single day and if I didn't have it 
I wouldn't be able to work, right creatively, keep up with friends, etc.

 Ride wheelchair cushion – –  I lost a year to pressure sores when every other 
wheelchair cushion failed me.  I tried EVERYTHING.  The ride cushion ( which, 
counterintuitively, is made of hard foam) is fully customized to each user. It 
got me back in my wheelchair and back to  work,  spending time with my kids, 
and enjoying life.

 Alexa for smart home control--  turning on lights, controlling my bedroom fan, 
calling attendants hands-free, listening to music,  ordering groceries, etc. 
etc. etc.it's amazing being able to do so many things by voice that once 
forced me to constantly ask for help.

 accessible minivan with EZ lock--  after my wheelchair and I fell off of an 
old school van lift, the ability to just quickly roll up the ramp and into a 
self locking  tiedown was revolutionary. I take it too much for granted.

 Fully tilting power wheelchair--  my Permobile  300's tilt function has 
extended my functional time in my chair and made pressure relief much less of a 
hassle. It's also been my most dependable wheelchair ever. It's going strong at 
10 years and has never needed anything more than batteries and replacement 
cushioning on my armrests.

 Lithotripsy kidney stone removal--  like many quads I have chronic kidney 
stones and this tech replaced difficult surgery. now, every 2 to 3 years, I go 
for a cleaning and I'm up at around in a week.

  iPhone/ iPad and Siri--  the touchscreen, and later the voice assistant, 
changed everything about communication, making me much more productive at work. 
apps made so many life management tasks incredibly easier-- banking, shopping, 
reading books, listening to books, tracking my kids, scheduling help, paying 
bills,  ordering meds, taking family pictures...  couldn't we all go on and on 
here?

 Viagra and Ferticare  vibrator--  getting erections and ejaculating aren't 
required to have a lot of fun in bed, but they sure are nice.

 wheelchair accessible RV by Newmar--  our first accessible RV was a DIY 
project that mostly worked, but wasn't all that comfortable. Newmar's  line of 
wheelchair accessible RVs were designed from the ground up for comfort and ease 
of use.  they are by far the best product on the market.  the family excursions 
it enables have given me some of my favorite memories


 thank goodness for these products and technology. I hope they stay on the 
market forever.  so, now it's your turn. WHAT IS YOUR LIST OF 
CAN'T-LIVE-WITHOUT PRODUCTS?

I've been using Peristeen since it first came out. My traditional program of 
suppositories and fleets gradually become more and more time consuming until it 
lasted for hours, was painful, and   triggered dysreflexia. it was also leaving 
me with bowel accident so often I was afraid to leave the house. Peristeen has 
been a complete game changer.  It's not as messy so using it is easier on my 
attendants.  it does not trigger dysreflexia for me.  it's not painful, unless 
I have a hemorrhoid (regular hemorrhoid cream the night before is taking care 
of that).  it takes the predictable amount of time and is comparatively speedy 
(about 45 minutes beginning to end every other day), so I can add it to my 
morning routine without it messing up my busy schedule.

Perhaps most importantly, it is EFFECTIVE.  It actually cleans me out and I 
almost never have bowel mishaps, anymore.  this has been a humongous benefit to 
going out, making travel plans (It's very portable, fitting in a midsized 
purse), my love life ( it not only eliminated untimely accidents it removed the 
mood to killing anxiety of potential accidents), etc.   I hadn't really 
realized how my old bowel management program had compromised my quality of life 
until I saw the difference Peristeen made.   In fact, I would say it was at the 
very top of the list of the products that have made my quadriplegia manageable, 
along with a Sure Hands lift,  a Ride wheelchair cushion,  accessible minivan, 
and Alexa as a smart home controller.

The only downside I experienced with Peristeen was its cost.  Since it wasn't 
covered initially by my insurance, I had to pay out-of-pocket around $300 A 
MONTH.  This was rough to handle but since it dramatically increased the time I 
had for work, and allowed me to go to the office without racing home from bowel 
accidents, it was worth it financially.  it took a few years but eventually I 
found a doctor in my network (Kaiser Permanente) that believed in the benefits 
of Peristeen  to advocate for it internally. Late last year, my insurance 
decided to cover it completely.

If Peristeen had not come out when it did, I probably would've ended up with a 
colostomy bag.  I understand they can work wonders for some people, but I was 
dreading it. Peristeen  gave me options, and a huge boost to my quality of 
life.  I rave about it to every Quad and Para I meet. my wife jokes that I'm 
trying to get a job at Coloplast (the manufacturer).

 I've been happy to see that Peristeen is is being strongly supported by 
Coloplast.  they just came out with an upgraded design that makes the reusable 
pump last longer.

Every time I have had chronic sweating it has been due to low grade autonomic 
dysreflexia (AD) cause by some source of irritation or trauma below my injury 
line. It can be the most maddening and scary situation to know something is 
profoundly wrong and have absolutely no idea how to pinpoint it and correct it. 
The fact that most healthcare professionals know little if anything about AD, 
or how debilitating it can be over time, always makes me feel even more 
helpless and hopeless.

Consistently, my worst trigger has been kidney stones.  For two years in the 
late 80s I was suffering both low-grade and flareup episodes of AD, and my 
inexperienced medical team did not pick up on some of the simplest warning 
signs, like chronic bladder infections and blood in the urine.  This was early 
in my post-accident years and I was too trusting of medical experts to demand 
that everybody aggressively pursue every possible source of the problem.   It 
almost cost me my life when I had post-surgery complications from removing two 
massive staghorn stones. Also, the   stones/infections/surgeries did 
irreparable nerve damage leaving me with chronic urethral burning – – which is 
the single biggest cause of suffering, depression, and reduced quality of life 
I experience.

Other AD triggers of mine I've had to track down include hemorrhoids, early 
bedsore breakdown on my heels, a spider bite, severe constipation, and fabric 
pressure points in my groin. Something like a dislocated shoulder or gallstones 
could absolutely be the cause of your sweating. You have to get those problems 
addressed (meaning treated!) to have any hope of tracking down the real culprit.

I've come to have a love-hate relationship with AD symptoms such as sweating, 
flushing, chills and body ache. On the hate side, they are immediately 
uncomfortable and distracting. If they persist, they become all-consuming, a 
constant source of worry and real physical distress. However, on the love side 
(maybe love overstates the case; maybe appreciate is more appropriate), without 
its warnings I would constantly be developing far worse physical problems. I am 
so in tune with that now, that the various combinations of AD symptoms can help 
me accurately discern between say, a moving kidney stone, and a hemorrhoid.

So, my advice is to please listen to your sweating. Don't stop until you 
eliminate, one by one, any source of irritation or trauma below your injury 
line.

Any one of those things could cause sweating.  For me, sweating is usually a 
sign of AD.  My gallbladder is full of stones too.  Has been for 35 years.  My 
doctor said, "ff you're ever really sick and they don't know why, tell them you 
have gallstones."  Gee thanks.  I know at some point, it will start causing me 
problems.  For now, if it ain't broke, don't fix it right?  Your shoulder 
situation is a little scary.  It could definitely cause sweating, but if you're 
not in pain... I wonder if the inaction on your shoulder is because you're a 
quad.  I think if you were able bodied, they would try to fix it.  Nobody wants 
to have surgery, but if it impacts your quality of life... Tough call.

Hey Jeff,

 I tested positive in September. I also have diminished lung capacity (half of 
my left lung is scar tissue from a pulmonary infection a year after my accident 
in 1985)

 thanks to the vaccine, my symptoms were very mild-- fatigue, shortness of 
breath, dizziness.  and I'm so thankful it wasn't worse.   for example, my 
wife's acute infection was much worse and she has some long covid symptoms.

 while I am sure I dodged a bullet, my primary attendant has pointed out that 
the shortness of breath and dizziness never went away completely. I still 
haven't been able to return to a regular exercise program because just a few 
minutes of weight work and the room starts spinning.

 I can hardly complain, all things considered. I really haven't told my wife 
and family. It would just make them worry and what good could it do?

I'm curious if any quads that got Covid had breathing issues before acquiring 
the virus? I lost all muscle tone in my diaphragm during my original injury 
and, naturally, could only build it back so much. I breathe on my own but 
probably have 50% or less lung capacity than the average able-bodied person, 
and Covid is a respiratory virus.

I haven't been on the quadlist for a while, but i'm checking in today.



I'm so sorry to hear about Lori. She was a nice woman who went through a lot. 
My condolences to her family and friends.



Somebody asked about quads and covid. I know three quads who caught it. All 
three got through it without being hospitalized, although one of them resided 
in a nursing home when he caught it.



I've been vaccinated and even went in for a booster third shot recently. I got 
the Moderna vax, and as discussed I had slight fever and yuckiness for about 24 
hours after the second shot. No such reaction after the third shot.

I just meant how many people are actually subscribed to the list.  I think most
