[QUAD-L] HEMORRHOID BANDING

2024-06-27 Thread Paul Jacobson
Hi, I have 3 hemorrhoids, one internal and two external.

I am considering banding the internal hemorrhoid.

Has anyone ever done this and what were the results?

How long did your pain last and would you do it again,

if need be?

Thank you,

RIDE SYSTEM: You get your butt and hips mapped and wherever there

Aae bony prominences, the areas where wounds occur, the cushion is cut out so

there is no pressure, friction or touching. These bony prominences, just
float

and are free from pressure.

The cushion is individualized to you. Hope this helps.

Can anyone explain why the Ride cushion is different/better? I'm currently in
bed because of a scrotum pressure wound, and I've been using a Roho Quadtro for
the past several years (after many more years on memory foam cushions). Thanks,
everyone, for your wisdom and good advice.

RE: [QUAD-L] CNA rates?

2024-01-29 Thread Paul Jacobson
Hi Aaron, I completely understand your point of view. In my 20 SCI years,

I found that it is best to pay more than the going rate. It lets me keep my

Caregivers for longer.

Turnover is the worst….losing that knowledge, having to interview and

retrain is just awful. I pay between $27 - $30/hr. It seems to work.

Thank you,

I know the rate that we pay our attendants can vary based on where we live, but
I have a hard time seeing more than $25 an hour being the top end. Especially
just for basic ADLs. If you are paying $30 an hour, I sure hope your
expectation of care is stellar. I mean immaculate care, always on time, doing
things above and beyond the basic or even more than that. I still pay today
starting at $15 an hour. It takes working for me at least six months before I
will bump that up to $20 an hour where I expect timeliness, thoroughness, and a
cheery attitude. If I'm going to go to $25/hr, they better do things beyond
ADLs. Things like vacuuming, mopping, organizing all of my things without
asking.

I think that what you may be feeling is "what would I want to be paid if I did
this job". You really have to get out of this mindset, because there are many
things I asked to be done that personally I probably would not do it unless it
was a direct loved one. Today most starting jobs are $15 an hour and they are
expected to do more work than what we expect for ADLs. I know it's hard not to
feel guilty, especially when they are a nice person and probably have a family.
People who we have even considered family because of how closely they work with
us. I have been through over 300 attendants in my 34 years. While I may have
felt very closely to those who stuck with me through years and invited to
family events and worked with the children. In the end, they were not actually
family. They had to move on and live their lives based on what they needed.

What may help is setting up a bonus plan. Set their pay and a base of $20 an
hour and list or just give bonuses each pay period based on stellar or above
the normal work. This would allow you to give a bonus of nothing or hundreds of
dollars if they were especially helpful one pay period.

Hope this helps, your brother in SCI.

Hi Lissette,

I am exactly like youhemorrhoids, back sleeper and spasms when I first lay
down.
I am in tons of discomfort during the BP. It always takes me 2 – 3 hours to
feel
better. After the BP, it always feels like I still have to go but that
resolves.

I take Miralax every other day, Benifiber each day, D-Mannose each day and
fiber
pills each day. Stay away from using creams for prolonged use, as this can thin
the
skin, which would make matters worse.

I did look into hemorrhoid banding but decided to pass. This could be a
solution.

Wish you well,

The method used to help clear bowels is Digital Stimulation, True most Quads
get hemorrhoids, some need to get them treated or removedso far I have been
okay with creams when they get bad. AS far as the problem when doing bowel
routine...many Quads told me that Miralax can really help and can be taken
regularly if needed, also was told fiber can help . Dan ***

Hello Carla, I recently discovered that I have hemorrhoids as well, I guess
from so much constipation and didge shimming. I also noticed increased spasms,
especially when I’m in bed, it’s not too bad, it’s usually just when I first
lay down and then they calm down. I think my mattress helps a lot, it’s a gel
mattress, however I do have a big butt which probably helps too. I can’t sleep
on my side because my shoulders start hurting after 15-20 minutes, so I sleep
on my back all night. I’ve tried hemorrhoid creams but didn’t help much. I read
that taking an anti-inflammatory could help, I haven’t tried it yet but I think
the best thing to do is to address the hemorrhoids. I think also fiber rich
diet might help. Also, not sure if the hemorrhoids are obstructing my bowels
from coming out, because I've been having the hardest time lately. Maybe seeing
a doctor for the hemorrhoid issue it's not a bad idea.

Do you have an air mattress? Because they are awesome for relieving pressure.
Most of the time I set mine to float but if I have a pressure sore I set it to
alternate and it will move air side to side kind of like you are doing with
pillow but more often and while you sleep.
I also have issues with hemorrhoids I spent 4yrs every 3 months having them
banded (rubber bands caught off blood supply) and they would fall off within 4
days. But Dr told me we would never get ahead this way so he finally referred
me to a surgeon to have them removed. That was at the beginning of Covid
unfortunately I am still on waiting list for surgery.
Anyway I am same level quad as you and my accident was in 2002 so almost as

drying out due to sleeping with blanket/pillow/etc over head or scalp not

After you get the vaccine, you're given the opportunity to have the CDC track
any symptoms you might develop. I get a text every day for 7 days with a link

The great thing about this country is that no one is required to get

Hi Paul,
I think you need to be wearing a more breathable fabric, especially in this
warm, humid weather. A couple years ago I had a horrible rash due to leaks from
a UTI, it lasted about 6 months. I tried all kinds of ointments, until a doctor
told me about Gelmicin. I buy it in Tijuana. It started working almost
instantly. I could probably send you some with my cousin Aleyda if you still
see her. Or do you ever cross to T.J.?

[QUAD-L] Myrbetriq for bladder spasms

Has anyone ever used this for bladder spasms? If so, thoughts?

Myrbetriq is a once daily pill that mellows bladder spasms but
doesn’t
affect acetylcholine; instead it quiets the bladder by
stimulating Beta 3
receptors in the bladders muscle cells and doesn’t cause
cognitive or
other severe side effects that often accompany anticholinergics.
It
received approval by the FDA for treating overactive bladder.

Why do people keep pushing on? I mean it's bad enough to be substantially
disabled, but it doesn't stop there. People, in general, don't want to be
around you. It's difficult to go places and do things; average things that most
people do all the time. And you know from experience that illnesses and such
can pop up at any moment, much more easily than the average person. You try
like hell to avoid being dependent on the government but it's nearly
impossible, even if you work full-time. That means you constantly have someone
with which to answer. Someone with their own rules, who can't begin to
understand you and what you go through on a daily basis. I could see where if
you were recently disabled then maybe you haven't been through enough to
realize what life is going to throw your way, but what about those of us that
have been around a while? What keeps you pushing on… Jeff

I've been cold since 1982, unless I'm in the sun. I moved to Phoenix for that

MS-Contin, as needed. My average body temp now is 96.5.

Though my body is falling apart now. I am just getting over UTI, Yeast,
Shingles, Infected Shingles, all at the same time. Still getting AD headaches
so next week I do a kidney and bladder CT scan, bladder scope, prostrate check.
Then in Jan I go to a rehab specialist, experienced with quads. I've been a bad
boy, It's been years since I've seen one and had any tests.

I've always had cold issues. I sit outside for hours when it's 110. Over that
and it gets to hot. The last few years if I sleep with my head lower than like
25 degrees, I shake uncontrollably cold. I would swear I'm freezing. But once I
put my head up, I feel better. No idea what's going on. I can get overheated
and still shake feeling cold, but put my head up and I warm up.

I also have a nerve issue in my left shoulder blade area. Thought is was a
sensitive mole, they cut it off. But it came back worse. Some days it's fine,
then for a week it feels like a knife is in my shoulder. Like stabbing pain,
yet a dead nerve going down my arm though my fingers. I've tried more pain
meds, lidocaine cream, patches, nothing helps. It hurts just resting there, but
lots worse when resting on the back of my chair or on my bed. I have to put egg
crate foam under part of my shoulder to keep it off the bed.

I'm hoping the rehab doc can find and fix something. Though I'm 51 (Wow, I
can't believe that) I feel I'm a 25 year old in a 100 year old body.

leave us itching. Regulate our bladder with ditropan, pain with Norco, spasms
with Baclofen? Here comes the dry mouth, dry eyes, itchy skin, flaky scalp,
etc. it's not an existential symptom like urethral burning, or social
sabotage like incontinence. I think I don't complain about it or mention it
much to my caregivers because compared to everything else it sounds like
trivial whining. But it is CONSTANTLY there in the background. Perhaps I have a
predilection for dry and itchy everything independent of anything disability
related. But, the impact of these essential drugs undoubtedly makes it worse.
I'll try some coconut oil, I'll try anything. Tea oil seemed to help for a
while but started causing acne. Sigh.

My doc suggested Crisco Oil. It helped quite a bit, but I found something
better... Coconut Oil. In a jar at room temp, it looks like sun tan lotion. But
in a real warm room it looks like baby oil. I rub it on my scalp, then wait an
hour. Then I scrub my head with one of those brushes that have little balls on
the tips of the teeth. Flakes fall off like its snowing. But, wow does my scalp
feel better.
And it also helps my dry legs.

nothing to do with your hands curling. Your body naturally wants to curl up and

Ok trying to be reasonable and rational here:

If he was sentenced to 150 years, he has not paid his debt to society. I
think he should be on house arrest - unless that means he would still get
650,000 dollars a year for care.

Could he still be a danger to society? What level is his injury? All quads
are not created equal... If he could be, shout it to the world - neighborhood
fliers, local media, national media.

The pissed off side of me (I've been dealing w/ my ex all day, so I AM
grumpy) says let anyone and everyone know that he is a RAPIST MURDERER!!
His injury has done nothing to exonerate him.

The calmer side of me just says, boy, is he in for a rude awakening when he
has to live like the rest of us w/ just mama for his pca. He might end up
wishing he was still in prison!

Bottom line... Amen Bobbie! It really yanks my chain to hear that a rapist
and a murder has gotten what I and most of you have been denied. He needs to be
retried by a jury of quad peers ... then he might get what he really deserves!

I had the spasms to. My urologist recommended going with a different

like $35 each. It's a Bard 22 French Bardex Lubricath Hematuria Foley
Catheter 30 cc Ribbed balloon Lubricious coated REF: 2550H22

This works well for me. Also consider meds for bladder spams (Detrol LA
and Doxazosin.

Good luck,

Detrol LA (Tolterodine) Tolterodine is used to relieve urinary difficulties,
including frequent urination and inability to control urination. It works by
preventing bladder contraction. Side effects: dry mouth, blurred vision,
upset stomach, headache, constipation, dry eyes and dizziness.

Doxazosin (Cardura) Doxazosin treats symptoms of an enlarged prostate
which include difficulty urinating (hesitation, dribbling, weak stream, and
incomplete bladder emptying). It relaxes the muscles of the bladder and
prostate. Side effects: headache, tiredness, swelling of the hands, feet,
ankles, or lower legs, shortness of breath, weight gain, muscle or joint

Subject: [QUAD-L] SP / Bowel Sweating

I've been having sweating issues for like 12 months now. Not sure if it's a
bowel or bladder issues, but I assume it's one of them. Nothing else makes
me sweat like this. At first I thought it was my SP incision. It was bloody
and prouding' (skin folding back on itself.) The sweating starts when I sit
all the way up. I thought having my abdomen fold right on that area was the
problem. But it has heeled up and it hasn't stopped the sweating. It really
got bad this week and this might be from having harder stool. That can cause
it too. But that's been fine for months and still the sweating. I know my
stool is not formed together well. I've been trying fiber drinks, but so far
no help. Anyone have issues with their intestines cramping and causing
sweating for no known reason? Sometimes it feels like I'm going, but nothing
is there. Gas can cause this in me and any sweating from a bladder issue
will cause gas in me. So I just can't find the cause, because any sweating
bowel or bladder causes the same feeling. I had my bladder scoped months ago
and everything was fine, but I'm going to ask for another one. I also had a
colonoscopy around that time and was fine. Though I don't want that again.

One thing about my SP, sometimes I get a very bad stabbing pain in my
abdomen, like double over pain. Can be stool, gas, but sometimes it's my
cath. It will be draining fine, move fine, but changing it and the pain goes
away. Like it's in a sore spot. Anyone else have these issues?

I just don't know why for many months I can only sit about 80% of the way
up, or I start pouring sweat. When everything else looks fine.

Thanks, Greg

That is way too long and have you ever been tested for a bone infection? I have
successfully conquered two of them, one of them my bottom and the other one
just below my elbow. You need to see in infectious disease for this and if it
shows a negative result. Then get a second opinion from a second specialist
just to make sure. I had to travel two hours from my place just to find a
doctor that would listen to me. There's only two wound clinics in my area, so
after visiting the one two hours away was able to get everything done just
under six months. The one on my elbow was that one I had to travel for and
nearly had it for nearly 2 and half years. I had to do my own research on
locating these doctors because my primary physician would not listen to me! I'm
on Medicaid and Medicare so I had no out-of-pocket expense for these doctors
except for my time. So the best thing is to do your homework and don't take no

I have a pressure sore on my tailbone that has been the bane of my existence
for years now. It is not deep and has never been seriously infected. However,
I have been fighting it for over six years! I have some feeling in that spot
and sometimes it feels like a red hot branding iron. Bad! It is about 4in by 4
in. In the last several days it has begun to bleed profusely. I don't know
whether this is good, bad, or irrelevant, but it is a first. Any of my buds
here got any knowledge to share about it? Thanks. Larry Willis

Hi Greg,

If it's a pressure sorethen you are getting pressure.without
question. If it's Stage 4
then I recommend...and this sucks..complete bed rest on an air
circulating mattress, start
drinking protein shakes and use a wound vac (they are amazing). 2 years
ago, I had a Stage

4was in bed for 5 months but healed completely.

I got a small sore just next to my tail bone. At first it was dime size and
shallow. It had that white/gray dead flesh in the center. The wound has
closed up size, but gotten deeper. We just start to get rid of the dead
stuff and then it's covered with it again. We pack it with some packing
strip stuff to keep it from closing. I have had a wound nurse coming out,
but now I'm going in to a wound clinic to see what's wrong. It's getting
real deep. we get it looking healthy, but then more dead stuff. I don't have
pressure on it, and no idea why it's doing this. It wants to close up, but
I'm told not to let it while it has dead tissue and is draining.

Dear Nicole,

I have always had more sediment or sand in my urine over the years that has
caused the catheter to block. If mucus is UNUSUAL FOR YOU... might not be a
bad idea to have your doctor order a urinalysis first and then a culture
displayed the results of the urinalysis. After that is done... a urologist or
just a doctor would be able to tell you it's something different is going on
besides the usual bugs that will always remain present for anyone who
indwelling catheter.

I don't think it will affect Medicare. If anything changes. It will
unfortunately get rewritten, changed, canceled, soon.

It should just be Medicare for all. You could opt out if wanted. The best and
cheapest way I think.

At least it makes them cover preexisting injuries. Though they mentioned For
kids so I hope it covers adults too.

I'd love to get secondary insurance to cover my 20%

I just can't understand the hatred over this, most from those who it would
help the most. The R's are just so good at scaring people out of their own best
interest. The big scary government, death panels, killing old people, etc.
Yesterday as congressmen were walking up the steps they got called Nig., got
spit on, etc. One R yelled Baby killer as a D was on the floor speaking. I'm
not happy how this Bill got put together or the Bill itself, but I never yelled
these things. Nor did I yell them at Bush ( Though I might have in the privacy
of my own home.)

Yes drastically. Our house was always known as the cool place-only
temperature cool. JMy grandkids always brought their sweatshirts when they
came to visit in winter. After my SCI I could never get warm until summer
came. However, the last few years have been much better. I thought it was
maybe only in my head, but a friend of mine who was around me a lot during the
first 4-5 years and then left for 14 years came back and remarked on how much
better I tolerate the cold now. I still like it warm, love my wood stove, love
the fact the Reno has 325 sunny days per year, but now that he mentioned it I
realize that I very seldom wear my heavy-duty poncho anymore.

I still prefer warm-up to 90° is perfect. After that temperature I get too
hot.

No, we lived in Reno before I broke my neck and there is no way I would move
away from my sunshine here. My whole house is built around sunshine coming in
almost all of the year, only in the middle of summer is the house not flooded
with sunshine.

I just figure my thermostat doesn't work well anymore. It takes me a long
time to get warm if I do get cold, and sometimes I will complain that my arms
or legs are cold when in fact they really are not.

I can tolerate the cold quite well as long as I'm sitting in the sun with the
sunshine on my face and there's not any wind-I was outside the other day for
two or 3 hours and it was only 50°, but I was sitting in a corner with the
sunshine on my face. We have a wood stove with a glass door so one can see the
flames, which makes me feel much warmer than just plain heat. My space heater
is also a radiant heater and I find that I'm quite comfortable with this setup
for 400 watts because of the glow. Mind over matter-or maybe if you don't mind
it doesn't matter.

  Good questions

  Joan

i'd just like to hear all the most current home remedies and medical
cocktails that have been successful with skin sores. My partner has an open
sore on one ischial, then the other one looks irritated and delicate. This
morning starts his third day in bed and he now has a red spot on one of his
heels (a pressure sore from being in bed all the time). Even tho we have an
air bed. We have always had great success with Silica Gel, high protein diet
including meets, eggs, and nuts (limit cheese), a protein shake, and sometimes
a circulation herbal supplement.

I'd love to hear what others are doing.

  Much Thanks
  Meghan

Subject: [QUAD-L] Offset Door Hinges

I only had the front door a door between my bedroom living room. All the
others had been taken off a long time ago. This morning I ran my wheelchair
into the door took it off the hinge. I knew the door was there. I knew I
was going to hit it unless I stopped. But the message never got to my hand.
At
least I haven't broken any screws in the wheelchair YET.

I am going to request offset hinges but I do not know anyone who has them.
Does
anyone here have them? The opening door frame to wall is small, only 4
inches.
