Yes, this is a royal pain in the butt. I had to drill out mine with a
very small bit.
I filed down the new ones slightly and applied anti seize to help the
next time.

I could have posted this exact comment 1 week ago.  Tossed those
shifters into my "I'll Deal With That Someday/Too Cool to Throw Away"
partsbox.  I'll never leave a cable head ungreased again.

The Campagnolo heads are slightly smaller.  That's why if you use
Shimano-compatible cable in an Ergo, you'll get the head stuck in the
shifting mechanism when you try to replace the cable.

I don't believe Silver shifters are set up for Campagnolo cables
though.

I began using Silvers when they first came out, & have used several
sets different bikes. I love them, but there is a consistent problem:
cable heads are too large to fit in the shifters. (I reported this
problem to Riv from the start, btw.)
To get around this problem, I file the sides of the cable "head", to
make it slightly smaller. Then the cables install and uninstall with
ease.
I have not used grease.

The Shimano ones appear machined differently and so do not have this
problem.  But I'd grease anyway.

Yes.  Twice.  Until the problem is fixed it's Shimanos for me.  (Yes,
the cable heads were greased.)
