That must be it. the cache.log is full of this type of entry:

[2007/04/08 20:41:45, 1] libsmb/ntlmssp.c:ntlmssp_update(267)
 got NTLMSSP command 3, expected 1

I followed a Mozilla bug report on this issue which mentioned the same
squid/gmail prompting issue I am experiencing:

It looks like it may be resolved in Firefox 2 so I have moved to 2.0.x
I also made several suggested changes to about:config in Firefox which
were suggested as helping ntlm and Firefox.

We will see how it goes.

Sounds like a browser bug to me. You should at most see one proxy login
popup at a time..

  It appears to be an issue with the ntlm type authentication
 because if I cancel out of the ntlm prompt and let squid roll into
 basic the prompts quit.

Not exactly unexpected. basic is a lot simpler and less demanding than
ntlm, and also cached aggressively by both browser and Squid.

 Browser: Firefox 1.5.0.10
 Squid: 2.6.STABLE9
 
 Any thoughts? Or is this yet another ntlm foible?

Seen Apache reports about NTLM being a bit broken in Firefox.

Hi all,
 I am having an intermittent issue with squid prompting multiple
times for authentication especially which using the gmail web

prompts back up so deep that I have to kill the browser process to
recover. It appears to be an issue with the ntlm type authentication
because if I cancel out of the ntlm prompt and let squid roll into
basic the prompts quit. However, both ntlm and basic use the ntlm_auth
helper. ie.

auth_param ntlm program /usr/bin/ntlm_auth --helper-protocol=squid-2.5-ntlmssp

and

auth_param basic program /usr/bin/ntlm_auth --helper-protocol=squid-2.5-basic

Here is some other info:

Browser: Firefox 1.5.0.10
Squid: 2.6.STABLE9

Any thoughts? Or is this yet another ntlm foible?
