The origins of the lines as a typographic trick probably bear some shared history with the use of underlining book titles as an authorial hint to the typesetter that the text should be italicized, though two generations of English teachers have since taught students to underline book titles, even though they were supposed to be italicized. In my case, though, I really did need the overline&underline option:
