If it is a PERFECT circular (or oval) hole.  An irregular one larger than 1cm
in diameter is probably not original MS.

I don't!!!  I got stuck in Tass Times in Tonetown for ELEVEN FUCKING YEARS! 
There was never any hint book, Michael Berlyn refused to answer my mail, and I
got generally frustrated (so frustrated that I refused to play his later game
Altered Destiny).  Only eleven years later did some dude offer to help me and
wrote up a walkthrough for me.

Tass Times... I think I paid my adventure gaming dues with that one.

If I ever take up Med Systems' Asylum again, the length of time will be
more like 17 years.

It looks like I might never ever find an IBM copy.  Does anyone even have
a copy?
