You have to select all the regions you need. Click on the little triangle in front and you can select e.g. individual provinces (?). You can select over multiple countries, etc. Press the info/help button in the right corner to get more info on how to select multiple regions. I doubt that is any admin border in OSM that you cannot download. The Dna study is for the entire Island of Ireland and Rory has since added a boundary download for the provinces to the ones that were already on offer. They have tried it and it is splendid.
