I'm using a newer version now and when doing the above I can choose
between sending to all in the list or just to every unique address.

That is exactly what I want to do :-)
In your example 5 messages would be sent out; the Smith family
would receive 2 messages and so would Alice and Thom.

Do you mean that you are trying to send messages to addresses like this:

Larry <[EMAIL PROTECTED]>
Kathy <[EMAIL PROTECTED]>
Nancy <[EMAIL PROTECTED]>
Alice <[EMAIL PROTECTED]>
Thom <[EMAIL PROTECTED]>
?
