With lots of help from Werner Icking, I published Bernardo Storace's "Selva
di Varie Compositioni" which extensively uses this sort of notation.
(http://www.pchpublish.com/gif/bspasap6.gif) . The macro \opennotes
redefines all the relevant macros so that after invoking it, you can enter
notes in PMX just as you normally would. (Well, except for the raised
half-rest). Then \closednotes returns things to normal. 

I had the opportunity once to meet Rinaldo Alessandrini, who has recorded a
CD of Storace. I showed him my edition. After leafing through it, his first
remark was "What program did you use to typeset the open notes?"
