Logout/Suspend/Shutdown button missing after upgrade to Ubuntu 12.04

Bug description:
Logout/Power off icon and other icons missing from desktop after upgrade to
12.04.
The error was due to the Theme setting in "Appearance" configuration being
left unitialized. The Theme selctor drop down menu was blank. Switching it to a
different them and then setting it back to the default reinitializes the
Appearance theme and all icons and buttons reappear. Please initialize the
Appearance theme to the actual default.
