Thanks Freddy, but I'm not interested in capturing the data and editing 
it like you could using OCR. I can see the form and  just  want to  fill 
in the blanks  and  print  them.  Most  image/graphics  editors  let  
you  place  a cursor  anywhere on image where you can start typing in 
text. This text just lays on top of what is already there, so you have 
to choose the right font size.  So I just want to get the bit map image 
of a rendered PDF file, use an image editor on it, and then print it.

pdftoppm and pdftohtml are command line tools that might be able to do
the conversion for you. The former says it can convert to ppm, which
can presumably be edited in something like photoshop.
