We did a temporary inplace-replacement with the 1.0.1 LTS clamav:

No, this bug is not fixed in 5.4.0-70. xen guests do not start with this
kernel version.

Until some days before we used selfbuilt apcu 5.1.11. After changing to
repo-apcu 5.1.3+4.0.10-1build1 several server crashed several times a
day. After changing to sury-repo
(5.1.17+4.0.11-1+ubuntu16.04.1+deb.sury.org+1) the problem disappeared.

All apache2 children died with "AH00052: child pid X exit signal
Segmentation fault (11)"

System: 16.04.6 LTS
PHP (before changing to sury): 7.0.33-0ubuntu0.16.04.5

Unfortunately these servers were live servers and we were not able to
reproduce or generate a backtrace.

Sonmething new? Do you need further information?

We tried kernel linux-image-4.15.0-54 and were not able to boot as pvh
(xen buld from source 4.12-staging from today).

linux-image-4.15.0-48 we are able to boot as pvh.

Some patches between 4.15.0-48 and 4.15.0-50 destroyed the ability to
boot as pvh.

while booting as PVH-Guest with Xen 4.12 (booting as PV works).

We have the same problem for Ubuntu 18.04 with Kernel 4.15.0-50. Ubunutu
18.04 with HWE-Kernel 4.18.0-20 boots without problems as PVH-DomU.

After installing / updating new intel-microcode-package
(3.20180807a.0ubuntu0.16.04.1 or 3.20180807a.0ubuntu0.18.04.1) we got no
microcode for Intel Westmere EP (CPUID 206C2).

After replacing /lib/firmware/intel-ucode/ with files from
https://downloadcenter.intel.com/download/28087/Linux-Processor-
Microcode-Data-File and update-initramfs -u the new microcode is loaded.

Did you ignore some CPUIDs?

Hi, we use Ubuntu as Dom0 and like this solution. But we need some
information if there would be solutions in the near future.

Ubuntu 16.04 use xen-hypervisor 4.6. So PVH, PVH shim or HVM shim can not be 
used. 
For 4.6 exists a mitigation "Xen PTI" (see 
http://xenbits.xen.org/xsa/xsa254/README.pti). 
Will xen-hypervisor 4.6 be updated? 

Will xen-hypervisor 4.9 (Ubuntu Bionic) get the patches for PVH, PVH
shim or HVM shim?

PVH seems the best solution. Will this supported by Ubuntu Bionic?

I patched the file mysql_ in /usr/share/munin/plugins/

This file is included in munin-plugins-core, see
http://packages.ubuntu.com/xenial/all/munin-plugins-core/filelist

I dont know what the .in-file is for, I also can not find it in source-git 
https://anonscm.debian.org/cgit/collab-maint/munin.git/tree/plugins/node.d

I fixed only this two bugs but tested today your linked version
https://raw.githubusercontent.com/munin-monitoring/munin/master/plugins/node.d/mysql_
(just replaced in /usr/share/munin/plugins/mysql_) and it worked without 
problems.

I did a short view on the diff between your link above and my patched version:
- the new version uses Storage (libstorable-perl is included in perl-package)
- some new values are given (e.g. replication)

So I think the use of the new version should be no problem. I would
prefer this new version.

It seems to be a bug in Kernel 4.4. (4.4.0-45)

After install ubuntu 14.04 with linux-generic-lts-xenial the same
problems like above occured (can boot linux, but cannot boot linux with
xen 4.4).

After changing the kernel from 4.4 to 3.13 (3.13.0-100) i was able to
boot with xen 4.4.

Public bug reported:

Installation of 16.04 was fine. Reboot and working without problems.

After installation of xen boot fail because the root-partition cant be
found. Booting the default-kernel works without problems.

Error:
Gave up waiting for root device

In /dev is no partition of the adaptec-controller Adaptec ASR8405.

I tried kernel-parameter:
- noacpi
- noapic (wont work with xen)

The firmware of  the raidcontroller is current (7.10 [33067])
