I must confess that I haven't read through the full history of this bug report, but I can confirm that it is still present in 12.04.1. This happens when the full screen application on the second display is not focused. As long as the application still has focus, the top bar (and the rest of the Unity interface if you have opted to show it) will remain hidden, but when the application loses focus it is drawn on top of everything on the display. Found the cause here. In /usr/share/themes/Ambiance/metacity-1/metacity-theme-1.xms: Section shadow radius is set to 8.0 just like in normal_unfocused.
