You have 3rd party packages that are the issue, eg.

Broken kxstudio-meta-audio-plugins:amd64 Depends on qsampler:amd64 < 
2:0.4.2-1kxstudio1v5 @ii mR >
  Considering qsampler:amd64 260 as a solution to 
kxstudio-meta-audio-plugins:amd64 0
  Removing kxstudio-meta-audio-plugins:amd64 rather than change qsampler:amd64

kxstudio-meta-audio-plugins is a 3rd party package, and has dependencies
that do not allow upgrade to occur.  Please also note this may not be
your only 3rd party package blocking upgrade.

Unofficial software packages not provided by Ubuntu can be removed using
a 'ppa-purge' tool prior to upgrade.

You filed the bug as related to `ubiquity` or the Ubuntu installer that
installed your system 2021-04-17 (109 days ago). That is the
responsibility of Ubuntu Desktop or Canonical.

Ubuntu-MATE however created and maintains the Software Boutique.  I
asked on IRC for an appropriate place and was given
https://github.com/ubuntu-mate/software-boutique which is where Ubuntu-
MATE track issues with their software. I suspect this is the v2 of their
software that isn't available yet for released products. There are no
issues being tracked there so I provided the link
https://github.com/ubuntu-mate/ubuntu-mate-welcome-legacy/issues (which
was the only link until semi-recently or for the 20.04 release (you'll
note issues reported there from this & last year)).

Ubuntu-MATE does not use launchpad for their own software, it's used by
Ubuntu software (and some other flavors). Software Boutique however is
Ubuntu-MATE software.

If you don't wish to use the Ubuntu-MATE bug tracker, I also suggested
you use the Ubuntu-MATE discourse site (see prior comment for link)

From what you pasted, your issue appears to be not with Ubuntu software
but 3rd party sources which is why I suggested support. You mention
`balena-etcher-electron` along with many other packages (and
dependencies) which are not Ubuntu supported as they are 3rd party.
Issues with those packages are tracked on the  bug trackers chosen by
their 3rd party creators

Thank you for taking the time to report this issue and helping to make
Ubuntu better.

You have a number of 3rd party packages that are stopping the upgrade,
eg. mesa-vdpau-drivers:amd64
(21.2.0~rc3+git2108022230.58bf0165ca0~b~mesarc5) which has a package
ver. that prevents upgrade to focal.

Please remove your 3rd party (Unofficial software packages) using a tool
like 'ppa-purge' from your PPA's and try again.

I've marked this incomplete, as third party packages from PPAs have
prevented the upgrade; the upgrade tools are intended to work with
Ubuntu repository software only. If you believe I'm in error, please
leave a comment explaining why and you can return the status to "New".

This is another "what works, what doesn't" Sorry (made b/c of focal
status at top)

But the
- sony vaio svp112a1cw (i5-9400u, 4gb, intel haswell-ULT)
boots the focal daily without issue, even current (20210729).

I've have only had issues with the impish daily.
(the install the prior comment was made from looks like daily 20210716 
according to https://phab.lubuntu.me/w/release-team/testing-checklist/ though 
I'll likely change that shortly)

So does this mean even SysRq commands direct to the kernel don't work to
allow a safe reboot, shutdown?  (if you don't recall the keys, a quick
search on a phone for "magic sysrq" will find
https://en.wikipedia.org/wiki/Magic_SysRq_key which is pretty good to
find the common REISUB statekeystrokes).

(Not being able to use SysRq to direct kernel directly shows the kernel
is a very bad state.  I read a RO file-system sometimes in your
description, so I'd also check the health of your hardware & explore the
logs and the messages before your "the file system is read-only"
mentioned issue.)

It appears you've added 3rd party packages that have prevented the
upgrade, such as

I would suggest removing such 3rd party packages and try again.

Today's boot contains 4 lines   (Lubuntu impish) on
- sony vaio svp112a1cw (i5-9400u, 4gb, intel haswell-ULT)

invalid image
failed to load
failed to load
... returns unsupported

the message is still very quick; the screen quickly being cleared and
"Attempting to decrypt master key..." from the last successful QA-test
install of Lubuntu impish on this box.

The thumb-drive boots normally on other boxes I've tested as usual...

The picture fits what @Leok describes I believe; which is different to
the sony vaio I used - however as the sony quickly moved to the next
boot device (ssd) it booted a prior installed image meaning I never got
a good look at it.

- hp 8200 elite sff (i5-2400, 8gb, nvidia quadro 600)

which shares the identical displays & thus setup (though I think
top/bottom are swapped due to cabling).. but I tried to test for that as
had top-display configured at bottom, and vice-versa...

either way I could NOT re-create the issue experienced on hp dc7700 on
this box; so it maybe related to video card !

This is likely a (duplicate) issue - the top of my lower display has now
become "warped".

GNOME no longer has a Activities/PANEL at the top of the PRIMARY display.
I'm chasing another failure-to-boot issue currently so not spending time on 
this sorry.

My display has ONE display (PRIMARY) below the secondary display
(secondary is at top)

the actual messages are more than I typed; but they only flash before
switching to the ssd boot which boots the prior QA-test install to the
box.

I've marked this incomplete as I believe insufficient details have been
provided.  What sort of crash?  Was it a kernel crash? did the session
freeze?  Did you take a photograph of the crash screen? if so where can
it be found?

the system using the same media, same box and run the `apport-collect`
from there. Please leave a comment saying you've done so (it will
explain why the attachments don't show a crash)

I see a number of 3rd party packages, esp. from
https://launchpad.net/~oibaf/+ppa-packages

For best results with a release-upgrade, you should remove all 3rd party
packages (bringing your system back to a pure Ubuntu one, ie. no 3rd
party packages) then perform the upgrade. The tool ppa-purge can assist
with this.

[The use of oibaf ppa] will prevent the upgrade as that PPA contains
package version numbers greater than the release to which you are
upgrading. You can revert the PPA by installing the ppa-purge package
from the official Ubuntu archive and then running 'sudo ppa-purge
ppa:oibaf/graphics-drivers'. After that you can try upgrading again and
if it still fails open a new bug report. Thanks and good luck!

this issue is not limited to Ubuntu..

I've been suspending debian-bullseye (11) on
- dell [optiplex] 780 (c2q-q9400, 8gb, amd/ati cedar radeon hd 
5000/6000/7350/8350)

lately and when I resume I have this issue.  The d780 box has numerous
lubuntu installs (used for QA & support) but I don't suspend any of
them.

Of note it's been a long time since I've had this issue with my
primary d960 box (it's on impish now as on development cycle).

Settings -> About shows a RED rounded-edge box with lighter-red circle-X

This will be what @EoflaOE refers to.
It's NOT what I saw on my primary install (which was a debian logo).

Either way FIX has been released
Daily has 1:40.0-1ubuntu1 installed :(

Installed update  (1:40.0-1ubuntu2)
Re-opened settings & UBUNTU LOG appears  :)

at the try/install query (maybe-ubiquity) I get video glitches at top of
wallpaper

changing wallpaper gets black background or glitchy-wallpaper (varying
each time) - ignoring that for now

VIDEO GLITCHES on wallpaper are actually appearing on the workspaces
display (may not be gnome terminology sorry); which I did not expect
(has it done that before? I've forgotten?).. in fact they're on the
screenshots??   Looking at bug report they've occurred before; maybe
it's just me not testing gnome very often (or not on this old slow box)

I don't get a forbidden message, or error.
I get a DEBIAN logo.  Given I like debian, I have no issue with it,  however 
that is not expected, and I bet not intended.

Ubuntu MATE impish daily QA-test (live) on 
- hp dc7700 (c2d-e6320, 5gb, nvidia quadro nvs 290)

A number of times, I've had windows open with the top title bar (will be
wrong term, but the close/minimize/maximize, program name etc bar)
hidden by the panel.

This doesn't feel right.

It makes it hard to move the windows around the screen. Likely an more
experienced MATE user knows shortcuts that will rectify this, but that
not me.

** Expected results

Given the panel is not auto-hiding, I expect windows to not be hidden by
the default panel.  This wouldn't be an issue if the panel was auto
hiding itself, but panel is still default (always visible).

** Actual results

Many windows are opening with the top bar exactly under the panel; thus
no CLOSE/MINIMIZE/MAXIMIZE buttons are visible nor window title etc..
Whilst I can read part of the window title using the bottom panel; it's
only part of it.

This makes it harder to move windows around (given the top bar is
hidden). Yes movement is still possible via MOVE on bottom panel via
right-click.

Picture shows ONE EXAMPLE only; it's happened a number of times.

On initial adjustment of Monitor.Preferences & position of displays
(instead of side-by-side, my top-bottom arrangement) the display
remained mucked up a long while before it settled.. ie. it looked like
this attachment.

Given time - all settled except the Monitor.Preferences issue this bug
is about.

Adjusting displays to match my setup (one screen above the other), and
at change the display became warped for a time, before correcting
everywhere BUT the Monitor.Preferences box.

I have two displays; ACER 22" + KGN 23"

** Expected Result

I expected to see my green KGN-23 above the redish Acer-22

** Actual Result

In the Monitor.Preferences box I now have TWO KGN-23 displays above TWO
ACER-22 displays

** Just a visual glitch

I can only move the LEFT KGN-23 & LEFT ACER-22 displays around; however
the image inside the Monitor.Preferences window remains incorrect with
shadows of the other displays still present.

(Apologies if incorrect package)

and today it worked :)
