How exactly is this fixed? Where?

The https://www.mozilla.org/en-US/firefox/64.0/releasenotes/ doesn't
mention it.

What is the opt-out mechanism?

This is WONTFIX.
Your argument is invalid.

No matter what you do, what facts or findings you present.
WONT.
FIX.
EVER.
(did you notice a decade passed since reporting this bug? A new generation of 
FF user came to existence since then! A DECADE!!!)

So I recommend to lock this bug, so people don't waste their time in
futile attempts to make some progress on it.

Nope. I am using 9.0.1 on Windows and after clicking a link the cursor
stays as it is (regular or hand, depending if it is over a link or not)
