Same behavior here. Middle Mouse Button (button2) not working, starting

xev | grep button

try it multiple times; button event occurs; stop xev and the button
keeps working.\

Environment:
KDE Neon (based on Ubuntu 18.04) with mainline kernel verion 
5.1.2-050102-generic
Hardware: 
Logitech USB-PS/2 Optical Mouse connected to a Lenovo Yoga X380 via Thunderbolt 
3 Dock

If someone else has those issues with a USB-C Dock (Thunderbolt 3) I got rid of 
the issue by using the latest mainline Kernel:
 5.1.0-050100-generic #201905052130 SMP Mon May 6 01:32:59 UTC 2019 x86_64 
x86_64 x86_64 GNU/Linux
and the following Kernel config settings (/etc/default/grub):

GRUB_CMDLINE_LINUX_DEFAULT=" quiet splash intel_idle.max_cstate=4
i915.enable_psr=0 i915.enable_dc=0 i915.enable_fbc=0
i915.enable_dp_mst=0"

Since I'm also facing this bug I just want to confirm that the error
also occurs on the latest mainline Kernel (5.1) and also on the current
drm-tip kernel version (5.1.0.0994 from https://kernel.ubuntu.com
/~kernel-ppa/mainline/drm-tip/2019-05-05/)

I'm trying to use an additional monitor on my X380 Yoga using an USB-C Dock. 
During the boot process the error message appears:
[drm:intel_cpu_fifo_underrun_irq_handler [i915]] *ERROR* CPU pipe A FIFO
After the boot process the monitor is sometimes enabled, but only a couple of 
seconds.
