Can confirm that for me it was caused by the clock on the top pager updating while using the place compiz plugin, for now have removed the clock (++rightclick remove from panel), that's stopped it popping up. I think the problem is with place plugin though and there's a bug report here https://bugs.launchpad.net/compiz-core/+bug/976032. [Bug 989663] Re: Totem does not play certain videos correctly.
Been trying to fix this same issue all week (totem opens mp4 files but then just sits there saying 'playing' with a track length 0.00/0.00), have finally installed vlc and that does indeed play them.
