I found the solution!!

To get rid of this annoying problem I 'purged' Pulseaudio, restarted my 
computer, installed pulseaudio again, and after that installed the 'volume 
indicator'.  
Sinds then (2-3 months  ago) its working perfect!!  It never worked for so 
long, even after a fresh reinstall I had the same problem after a week.

So for those of you who don't understand:

sudo apt-get purge pulseaudio -y
Restart
sudo apt-get install pulseaudio -y
sudo apt-get install indicator-sound -y
restart

I saw this "[ 241.848195] 2:3:3: cannot set freq 16000 to ep 0x86" appearing in 
my 'virtual terminal'  when I switch to it using'crtl+alt+f1'.
I don't know what this means, or if its a problem in the first place. so I left 
it the way it is.
