Public bug reported:

A document with endnotes, attached, exhibits two problems:
1. The note number of the first endnote is formatted differently from the 
others, and if there's a way to change it, it sure is not obvious -- I can't 
even select that number.
2. The endnotes are forced onto a separate page.  Again, if there's a way to 
change this, I haven't found it.
