I have idea on this problem: looks URBs not unlinked from queue before
cleaning halt.

If you'd like to test, I will build a precise kernel with fix...

Step to reproduce:

Attach an android phone using USB. Turn on the USB modem feature. The
module will crash randomly.

To esatblish connection, I can reset the phone or the computer, or both.
