I had something similar happen to me. I was using a custom .gtkrc-2.0 file in my /home that I had forgotten about. When I cleared it out, the problem went away. Ca you please provide more information about your installation (update or fresh install). the index.theme file points to Human metacity theme so I am not sure why this is happening.
