Close firefox, then reopen, the icons are all gone. The icons for back, forward, refresh, home, stop are NOT visible, the folder icons for folders in the bookmarks toolbar are all visible. I seem to be able to replicate this time and time again. sometimes the icons load - especially (100%) after installing a new extension and restarting firefox, but most of the time the icons are missing...
