This happens here with many extension icons (Adblock Plus, Video Download Helper etc.). The Custome Toolbar dialog sometimes shows the missing icons so I can add them but after a firefox restart they are gone again. If I use the restore default set button in the dialog, all additional icons introduced by extensions are gone from the toolbars and the dialog! Workaround: Things seem to be stored in localstore.rdf in the firefox profile.
