Reviewing your log files attached to this bug report it seems that there is a problem with your installation media (CD/DVD) or with your hardware. Please retry your installation with new media and if it still fails I'd recommend testing your memory. You will need to attempt to install Ubuntu again in order to get the log files. Run the installer until it crashes, and then open a browser window and attach /var/log/syslog and /var/log/partman to this bug report following http://wiki.ubuntu.com/DebuggingUbiquity/AttachingLogs .
