I understand and agree with your assessment, and can see that in the normal 
scheme of things there would be a .old file.
But evidently something chose to modify it itself and chose not to make a 
backup, and there's no obvious way to insulate against that.

(unless we made these critical files object-orientated in some way so
we could police access to them (so every update through the interface
forced a backup to be made first)... rather like the registry or vpd of
other OSs)

Re Makoto... yes, I was hoping to find a .undo file.. but there wasn't
one in this case, but your problem sounds more likely to be alacarte
related.

Makoto - you have a different problem. Your problem is that Alacarte
appears to have corrupted your users local applications.menu file. The
problem in this bug report is that some package appears to have
corrupted the system-wide applications.menu file, which is a completely
different issue. Alacarte doesn't even touch the system-wide menu
configuration. If you still have the corrupt file, then you need to open
a new bug report really. Without the corrupt file, there isn't much else
that can be done.

Andy - I'm not sure where to go from here really. There isn't really any
chance of finding out what caused your problem, unless it happens again.
If there is a particular package that is corrupting the
applications.menu file, then a bug report needs to be opened against
that package. In the meantime, I'm going to close this report for now,
as this isn't a bug with gnome-main-menu, and I don't have a package I
can assign this too.

If you experience this problem again and have an idea which package is
causing it, then you can re-open this bug report by adding that package
as a task, by clicking on the 'Also affects distribution' link above
(below the yellow bar). Once you have clicked this, make sure the
'Ubuntu' distribution is selected, then enter the appropriate source
package name.

In the meantime, I shall stay subscribed to this bug report, so it won't
get lost.

I can confirm this bug.  It happened to me for about 45 minutes, while I
scrambled for a meaningful solution.

I don't know many details, but I *believe* that I did use Alacarte to
edit my menus.  After it's all said and done, POOF, no more
Applications.

Anyway, here's the steps I took to solve it.

1)  ls -al .config/menus/
2)  Look at the most recent applications.menu.undo-x
3)  cp .config/menus/applications.menu applications.menu.backup (this will just 
back up the borked menu file to your home directory, just in case
4) cp .config/menus/applications.menu.undo-x .config/menus/applications.menu 
(copies the newest applications.menu.undo-x over to be your default menu)

It should work after that, but that's the solution I found.

that sounds like a bug, then? (the extra /menu ...

I realise this is very hard to track down... what I was thinking might
be helpful, though, are things which would mitigate the disaster if it
happens again... e.g. have the system make a backup of key files like
applications.menu before it makes a change which substantially alters
its size.

Or ask the user before making drastic changes to vital files... after
all, it's not likely that I'd want to erase my applications menu, so if
it looks like it's about to happen, maybe check with me first? After
all, it didn't actually uninstall all my applications, so there has been
a disconnect in the process somewhere, where it removed all the icons
from the menu but didn't touch the apps (thankfully!)

I'm not sure how to get a single file out of a package.. but I'm sure
google can tell me :) Thanks for the info.

Your applications.menu file (which describes the structure of your menu)
belongs to the package gnome-menus. If gnome-menus is updated and the
new applications.menu is different to your old one, then Apt will ask
you if you want to install the package maintainers version, or keep your
existing version. It will also allow you to see the difference. This is
true for any file described as a configuration file in the deb package.
If you install the package maintainers version of the file, then a
backup of your old file is automatically saved (with '-dpkg.old'
appended on to the filename).

The problems arise when another package makes changes to a configuration
file in a preinst or postinst script. If that application messes the
file up, then it is a bug in that particular package.

However, the /etc/xdg/menus/applications.menu should never need to be
modified by any package - ever! And I'm not aware of any package which
actually modifies it. The problem with modifying it to add menu
categories, is they will probably get lost during an upgrade of 'gnome-
menus', as most people will install the package maintainers version of
the file (which will restore the default menu structure). If the
applications.menu is modified by any other package, then a bug report
should probably be opened against that package, as it doesn't need to
modify it. If a package needs to add additional categories to the
Applications menu, then it should place a '.menu' file in the
/etc/xdg/menus/applications.menu-merged folder.

An example of this type of package is Wine, or Crossover Office. Both of
these packages add a new category to the Applications menu. They both do
this by placing their own '.menu' file in /etc/xdg/menus/applications
.menu-merged/

I've had a look through your logs, and I can't find any suspect package
that might modify your applications.menu. I'm really not sure what to do
from here.

I was installing libdvdcss using these instructions...

at some point in this, it said it couldn't install raop-play, and stopped the 
installation.
(this was probably because I had already installed raop-play previously)

Then the Update Manager icon on the top right icon bar turned to a stop sign 
(white bar on red circle), so I clicked it, and it said a partial installation 
had happened, and would I like it to clean up. Said yes.
Then it said would I like it to delete the things it had cleaned up... said yes 
(probably my big mistake, but seemed reasonable at the time!). 
It then chugged away for several minutes (was getting a bit suspicious).

When it finally completed, Lotus Notes had been uninstalled (that had
been installed somewhat manually), and the Applications menu just gave
the few little white pixels in the left corner that happens when there's
no applications menu.

I have attached the ~/.config/menus/applications.menu and the
/etc/xdg/applications.menu that I was left with after this.

I read somewhere that deleting ~/.config/menus/applications.menu makes
System-Preferences-Main Menu (alacarte) create a new one, so I tried
that, but it did nothing..

that was what cause the alacarte error I listed above.

Copying in an applications.menu from a friend (also on Hardy) didn't
help either (applications menu was still empty). This did start in
alacarte, but it just had templates for the main folders - nothing in
them, so I'd have had to repopulate the entire menu by hand.

I hope that gives you some more clues - I realise this is a weird one,
and I have no real idea how to reproduce it.

Your /etc/xdg/applications.menu has an extra '/Menu' at the end, which is 
probably why it doesn't work. This is backed up by the error you got when 
running alacarte:
'** (alacarte:3340): WARNING **: Error loading menu layout from 
/etc/xdg/menus/applications.menu: Error on line 174 char 8: Element 'Menu' 
was closed, no element is currently open'

Could you please attach your /var/log/dpkg.log and
/var/log/apt/term.log?

To be honest, it is going to be very difficult to work out what happened
to your menu configuration.

FYI, if you need to restore the menu configuration, then
/etc/xdg/menus/applications.menu is part of the 'gnome-menus' package
