I assume this is connected with the ALSA sink. Happened to me on amd64
several times now, the last time playing music from Songbird through
GStreamer (thus PulseAudio native) and VoIPing with Linphone (ALSA Pulse
Sink).

Also, after this happens, I can get sound back by doing:

$killall pulseaudio
$sudo /etc/init.d/alsa-utils restart
$pulseaudio

but my volume controls do not work (Volume buttons on keyboard that work
before the crash).  It does indicate a change in volume (with the   I
can:

$killall gnome-volume-control-applet

and restart it, the notifyosd popups indicate a change in volume level,
but the volume does not actually change.

Thanks for the report, could you get a valgrind log of that  crash? Please 
follow this procedure:
set pulseaudio to not respawn:

echo autospawn = no|tee ~/.pulse/client.conf
killall pulseaudio

and get the valgrind log running:

G_SLICE=always-malloc G_DEBUG=gc-friendly valgrind -v --tool=memcheck
--leak-check=full --num-callers=40 --log-file=valgrind.log pulseaudio

After getting the log, please attach it to the report, Thanks in
advance.
