Are you per chance performing your tests with ekiga, as that is the SIP client 
that happens to be at hand?
In that case, this bug might actually be bug #480207.

Thanks for reporting this.  Can you provide a log from a verbose
asterisk console?

$ asterisk -r
place call

Upload the contents.

I haven't looked too closely at your configs, or tried them.. But my first 
thought is the change in 1.6 defaults:
* The autofallthrough setting introduced in Asterisk 1.2 now defaults to 'yes';
  if your dialplan relies on the ability to 'run off the end' of an extension
  and wait for a new extension without using WaitExten() to accomplish that,
  you will need set autofallthrough to 'no' in your extensions.conf file.


Additionally, can you try the new Asterisk package uploaded today?
