Please attach /etc/default/console-setup.

Is it possible that you installed the beta and haven't upgraded since?
This sounds awfully like bug 421212, which was fixed just after the
beta.

Binary package hint: debian-installer

This functionality is working in the Debian Debian installer, but not
Ubuntu Karmic Koala.

I select dvorak at the boot prompt and then later during the installation 
dvorak or dvorak international.
When installation is complete, the GDM login is QWERTY layout.
I believe the VTs are dvorak, however.

I guess that the major category is used but the variant (Dvorak) is lost.
Maybe this is due to no longer having an xorg.conf file and the information not 
otherwised being passed to or retrieved by the X server.
