you might actually be hitting bug 691651 in unity which should be fixed with
today's upcoming update to unity

I tried this in evince / Natty alpha.
Appmenu (File, Edit, View etc.. ) was only visible on mouse-over, which is
good imo.
The text displayed in its stead was document viewer, the descriptive name
of the Application.
This is foobar, since it doesn't give the user information about *what*
document is being presented.
After a mouse over to the appmenu area, the application name
disappeared, and the document title was displayed instead, which makes
sense.
i am guessing this happens only when a document is first opened. After the
first mouseover to the AppMenu area, the document title is shown instead.
AIUI the app name should *never* show up in the AppMenu area, since we want
the system to be invisible and to just work.
An app name in this respect would be a part of the system, but the
document title or name would be information relevant to the user.
