Oh ok, the one you filed is good.  Not sure who works on that package though.  
If it goes stale over time and no one touches it, pop into #ubuntu-bugs in IRC

You are running lucid but have precise sources for some reason
Possibly/probably because the do-release-upgrade -d fails and does not 
complete... I think this is caused by an ldap upgrade failure:

Well you are correct.  It looks like sldap is the reason for your
problem proceeding.

Errors were encountered while processing:
 /var/cache/apt/archives/slapd_2.4.28-1.1ubuntu3_amd64.deb
E: Sub-process /usr/bin/dpkg returned an error code (1)

I would suggest filing a bug against that package.  This report is rather large 
now so it would not be the best for me to change packages at this point.

I wouldn't expect the problem to pop up on a freshpa's 10.04 without any
additional packages installed. But all the packages on my system are
from the repository. No ppa's, deb's or custom compiled software was
used...

What I find odd it that your getting a duplicate error in the sources
file.  That should definitely not be the case with a fresh install.  Had
you release upgraded that machine in the past?

Also attach the most recent logs. (/var/log/dist-upgrade)  Looking at your 
older ones, I didnt see an error in regards to upgrading.  I understand you 
were getting that candidate message but it was not throwing up any error 
messages in the logs.

Maybe this is caused by the do-release-upgrade failing (a.k.a. we are
testing on a broken system)? I could retest this from the begining (as I

Well now, if you like, we can clear out the old logs and try a do-
release-upgrade again.  If it fails you can attach the new logs.  I just
wanted to get rid of the 'No candidate ver:' errors and they look like
they are gone.

If you want to clear out the old logs run:

sudo rm -r /var/log/dist-upgrade/*

The release-upgrade does not go well. Isn't it better to try to do a
release upgrade from the image before any update to precise? I have the
idea that it got borked as I let the upgrade continue with these missing
candidates.

Hhmm, I run into trouble... I start with a clean 10.04 server. when I
run do-release-upgrade -d I get the missing candidate warnings so I

I then run do-release-upgrade -d again but I still get the missing
candidates warnings...

I have a server running and when I installed it years ago I also installed an 
virtualbox image with the same packages... I test updates on this image before 
updating the actual server...
Recently I was testing a do-release-upgrade on this image and I got these 
warnings...
I did not install any packages that where not part of the repository

Well it seems to be a local error.  I have just finished installing a fresh 
10.04 and updated to 12.04 without issue.  I would suggest opening a thread in 
the 'Answers' part of launchpad, askubuntu.com, or the ubuntu forums.  They may 
be able to assist in a more timely manner fixing your problem.

I just tested this on a clone of the server and it is still an issue:

I am interested in what an update will look like.  Are there any errors
in an apt-get update?

If you want, you can run the command then copy and paste the output to a file, 
and attach that file here.  I do see that duplicate error.  We may have to 
clear that up to fix the issue.
