The WARNING: Failed to read mirror file seemed to stop me from using upgrade-manager -d, but on the other hand do-release-upgrade -d seems to work. I still get the warning, though, and I'm running the upgrade right now. With upgrade-manager, it never started to download files after the warning message came. I'm trying to upgrade from 10.04 LTS to 12.04 LTS.
