I have lost access to the hardware that this problem was happening on (I no longer work at the company that owns the hardware), so I will not be able to troubleshoot this further. Alexey, this indeed has nothing to do with NetworkManager. Second, on one of the ~20 reboots since the problem started, the interface came up OK and did not need to be started manually, so it seems that the problem is intermittent.
