Public bug reported:

Currently running with nouveau driver.

After activating nvidia-current driver trough additional drivers I did a
reboot. The system then fails to load the graphical driver, and freezes
completly. I'm not able to get into a shell or anything. The system
powers down when pressing power button.

To fix this, I booted in failsafe mode and removed the nvidia-current
driver via root console.

Phil,

Did you simply patch source / compile / drop in?  Haven't (bothered to)
compile any source since moving to Ubuntu dapper.  Not sure if there's a
ton of other dependencies that need a recompile in order for this to
work.

As X is started for each user, control can be returned to the actual
machine by killing that particular X session via SSH once the user has
attempted to log out.

ALL input via the keyboard and mouse is completely ignored until X is
killed. Even the CAPS LOCK button doesn't work.

I cannot find anything in any of the logs that suggest there has ever
been a problem. Perhaps I'm looking in the wrong place?

I have no idea why this has been sent to the fglrx queue.

Looks like Alberto has already uploaded 304.51 to quantal.  If the bug
is not in pointer barriers, or if it is but is not immediately fixable,
maybe we should consider keeping nvidia-common to 304.43 and move 304.51
to nvidia-current-updates.

The numbers 1 through 8 missing from Hebrew (lyx) keyboard layout. This
means that this layout is no longer usable.

To reproduce:
1. Add Hebrew (lyx) keyboard layout.
2. Switch to this layout.
3. In gedit, try typing a number "12345678" from the numeric row (not the 
keypad).

Expected results: the numbers should show up.

Actual results: nothing is shown.

Using the layout viewing option in the control centre, it can be seen
that the numbers are missing from this layout.

The faulty definitions are found in the file:
/usr/share/X11/xkb/symbols/il

Looks like precise ships 295 in -updates.  So I think this bug can be
closed out as fixed.  I agree updating to 304 makes sense now but bug
1037483 tracks that.

This problem disappeared and reappeared and disappeared and reappeared
and disappeared for no apparent reason, until about two weeks ago. Since

Reinstalling all of my mesa packages borked my DRI:

Reinstalled 96.43.23 & got DRI (and GL vertex buffer object) back.

@Flames:  PEBAC? Were you trying for PEBKAC or similar?

I realize what vdpau is used for. However:

$ apt-cache depends mplayer | grep libvdpau
  Depends: libvdpau1

@giardia: libvdpau1 error now appears on the 96.43.23 that I did. This is due 
to reinstalling libvdpau1 before the reintall:
$ cat /var/log/nvidia-installer.log |grep libGL
 /usr/lib/i386-linux-gnu/mesa/libGL.so.1 -> libGL.so.1.2 (File
WARNING: Unable to restore symbolic link /usr/lib/i386-linux-gnu/mesa/libGL.so

Ok, yeah, looking at the code again this should no longer be able to
happen.  The fix was in 3.1 (See bug #1041104) and we're on 3.2 by now,
so it probably went away when you updated.

vdpau is only ment for newer graphics cards. It was not even invented,
when these cards were produced.

problem still occurs - only now the computer doesn't hang - but
compiz/Unity crashes, and i get left off with plain windows with no

For GPU lockup bugs with Intel graphics, you need to collect the output
of 'dmesg' and your /sys/kernel/debug/dri/0/i915_error_state file.  Both
of these must be collected while the machine is locked up (e.g. by
sshing into the sick machine over ethernet).  See

@giardia: Interesting... I had not noticed because I hadn't run mplayer
since the previous install of 96.43.23. So my log doesn't show the
error. Now that I've reinstalled libvdpau, I suspect that the next
reinstall of 96.43.23 will show the same.

However, I do see these:

Possibly this is not an Xorg bug, but when I log in X appears not to
start and bounces me back to the lightdm greeter.

I tried Unity-2d and Unity with the same result.  I don't see any errors
in the Xorg or lightdm log, but there may well be clues I'm missing.

I just pulled in the latest updates with an apt-get dist-upgrade, which
seemed to be a dbus security update, and then rebooted.  I tried logging
in with the most recent old kernel, no improvement.

Looked to me like ubuntu-bug was looking for the wrong files in
/var/log/lightdm, so I'll drop in the correct ones.

Just added a Microsoft Laser Mouse 5000 to my pc and am now having this
issue also...however I believe I may have found the cause (although I've
only investigated briefly).

Having downloaded the xserver-xorg-input-evdev-udeb source package
(http://packages.ubuntu.com/precise/xserver-xorg-input-evdev-udeb) the
changelog contains mentions of merging a smooth scrolling branch on Wed
Nov 9 16:01:48 2011 +1000.

evdev.c contains conditionally compiled code depending on whether smooth
scrolling should be enabled as to how it handles the scrollwheels.

Either:

In EvdevProcessRelativeMotionEvent we have:

Before smooth scrolling, REL_WHEEL was mapped to vertical scrolling and
REL_DIAL and REL_HWHEEL were mapped to horizontal scrolling. After
smooth scrolling REL_WHEEL and REL_DIAL are mapped to verical scrolling
and REL_HWHEEL is mapped to horizontal.

Fixed for me by changing the REL_DIAL axis to SCROLL_TYPE_HORIZONTAL.

I've attached a patch for the source which solved it for me. Suppose
alternatively you could just remove the HAVE_SMOOTH_SCROLLING
definition.

I have similar behavior.  I am on 11.10, and have seen this for a couple
of years (certainly 10.04, maybe 9.04).  For me it is a keyboard and
mouse plugged into my ThinkPad docking station that die.  If I plug them
into my laptop's ports, they work fine.  I also occasionally have my
laptop's keyboard and trackpoint die in the same way, in which case I
have to reboot.  All works fine after reboot, at least for a while.
Like the OP, it's random and hard to reproduce.  Unlike the OP, I've
never once had it start working again.  lsusb -v segfaults when this
happens:

Note the warning after the first line of text.  Sadly, it doesn't say
which device it is looking for.

When this happens, there is a light on the lid that turns on.  It has a
square with three dots in a triangle shape in it.  That light never
lights for any other reason.

Currently the audio port on the laptop (but not the ports on the dock)
do not work, speakers do work, and the camera does not work.  I suspect
this is because of the USB problem.  Generally they work fine.

Finally, I am on a ThinkPad W510, and was on a T61 before this with the
same problem.

It's probably worth pointing out that you had to define an xorg.conf
because you got "No devices detected" in the Xorg.0.log.  A similar
problem to what is currently happening to some (all?) radeon cards with
PowerPC at the moment since the latest update to the radeon driver - bug
1058641 .

python3 unicode strikes again.

Some people's Xorg.0.logs are in UTF-8 (or other non-ascii formats), but
apparently yours is still plain ascii.  So the logic there needs to cope
with a multiplicity of Xorg.0.log formats.  (Presumably if you convert
it to UTF-8 the bug filing will work.)

my computer will freeze and stay frozen until i unplug it when i do any
1 of these two things:the first is it freezes roughly 90% of the time
when i drag a program into the corner to make a half window, the second
it freezes roughly half of the time when I select from one of more than
one instance of a program and it maxamizes.

The version of ubuntu im using is 12.04.1 Lts 32 bit version.

it is not program specific it will happen on any program i attemp this
with.

 1. the fact that the GPU hangs in the first place
 2. the fact that the kernel is not able to recover

The first bug is probably in the xorg radeon driver and the second one
in the radeon DRM driver in the kernel.

I just tried again and it seems that this bug is at least partially
reproducible:

 - the GPU hangs occurred again after a few seconds and the kernel was not able

I do have this bug also since some time (ok, some months, maybe over a year). 
Recently I stumble upon Bug#926906 and Bug#712584, both on Firefox accessing 
the grafix card and AppArmor blocking the access. Since all my crashed seemed 
to be preceded by a ratelimit and some audit denies (like the poster above 
stated) I reasoned, if the bugs are connected in some weird way, lets say 
AppArmor block firefox from accessing /dev/ati/card0 and the overflow on 
ratelimit crashes the system ?
At least I now adapted my firefox apparmor profile and gave it access to 
/dev/ati/card0. The audit messages are gone and so is the ratelimit msg. I'll 
watch the situation and post back, if my crashes are gone.

For NVidia the bug was fixed with a new abstraction for apparmor.

Note that apport wrote this in my bug report:

but this does not apply to the moment of the crash.  At the time of the
crash, glxinfo would have displayed this:

OpenGL vendor string: X.Org 
 
OpenGL renderer string: Gallium 0.4 on AMD RV635
 
OpenGL version string: 2.1 Mesa 9.0-devel   
 
OpenGL shading language version string: 1.30
 

This is because I did not restart the computer before reporting this bug
(and probably because of the GPU hang, xorg decided to fall back to the
llvmpipe software renderer).
