computer's buggy and outdated BIOS is available (A05). If you update to
this following https://help.ubuntu.com/community/BIOSUpdate does it
change anything?

If it doesn't, could you please both specify what happened, and provide the 
output of the following terminal command:
sudo dmidecode -s bios-version && sudo dmidecode -s bios-release-date

Please note your current BIOS is already in the Bug Description, so

Sorry for the delay — was only able to find time to reboot yesterday.

Daily live (from about a week ago) seems to help. I was able to change
display position, log out and log back again from the live usb (without
installation).

Could you please test the latest upstream kernel available from the very
top line at the top of the page from http://kernel.ubuntu.com/~kernel-
ppa/mainline/?C=N;O=D (the release names are irrelevant for testing, and
please do not test the daily folder)? Install instructions are available
at https://wiki.ubuntu.com/Kernel/MainlineBuilds . This will allow
additional upstream developers to examine the issue.

If the latest kernel did not allow you to test to the issue (ex. you couldn't 
boot into the OS) please make a comment in your report about this, and continue 
to test the next most recent kernel version until you can test to the issue. 
Once you've tested the upstream kernel, please comment on which kernel version 
specifically you tested. If this issue is fixed in the mainline kernel, please 
add the following tags by clicking on the yellow circle with a black pencil 
icon, next to the word Tags, located at the bottom of the report description:
kernel-fixed-upstream
kernel-fixed-upstream-X.Y-rcZ

Where X, and Y are the first two numbers of the kernel version, and Z is
the release candidate number if it exists.

Thank you for taking the time to work on this, I updated my bios version to A05 
as you requested but the issue still occurs. The output of the command you sent 
is:

The quickest way to reproduce this is to:

1 -Connect to an external monitor.
2 -Run dual displays(laptop monitor and external monitor, both 1080 in my case).
3 -Open http://www.twitch.tv/dotamajor on laptop screen and run it at full 
screen. any fullscreen stream works but that is what I was running at the time.
4 -Click anywhere on the external screen after the full screen starts streaming 
video.

This only started happening after the upgrade to 15.10

An xinput set-button-map alternative can be used, but is less dynamic
(it will not be applied automatically when your device is detected). The
following script can be configured to run at startup (in Ubuntu there is
a Startup Applications GUI to set it up). For some reasons it won't work
if you simply call the script from your ~/.profile file. The MOUSE_NAME
variable corresponds to whatever name is given to your mouse in the
output of the xinput list command.

No matter where I put my custom mouse input device xorg config, I cannot
get the settings to be used. I tried putting my file (90-razer-
lachesis.conf) in various places (/etc/X11/xorg.conf.d/,
/usr/share/X11/xorg.conf.d/ and even copied the content of my file into
/etc/xorg.conf), but nothing works.

The custom config seems to be picked up by by Xorg, as can be found in
the /var/log/Xorg.0.log file, but nonetheless the configuration defined
in the config is not respected.

Here's the config file (90-razer-lachesis.conf); it should merely
inverts the left and right buttons, to be used as a left hand mouse:

I'm also having this issue regularly, at least two times a day.  I can't
say for sure but it seems that opening/closing tabs in Chrome is what
triggers a crash most frequently here.
