problem does not seem present.

I chatted with tjaalton in irc. He suggested trying mainline kernel.
I pulled http://kernel.ubuntu.com/~kernel-ppa/mainline/v4.7-rc6-yakkety/
now running 4.7.0-040700rc6-generic
with
$ dpkg-query --show xserver-common xserver-xorg-core
xserver-common  2:1.18.4-1ubuntu1
xserver-xorg-core   2:1.18.4-1ubuntu1

and the problem does not appear to be present.

I can't run the mainline kernels due to need of zfs and other ubuntu
sauce, but it does seem fixed there.

I backed xserver-common and xserver-xorg-core previous versions and am now fine.
this works:
$ dpkg-query --show xserver-common xserver-xorg-core

the latest versions (1.18.4-1ubuntu1) do not.

Previously (with 1.18.4-1ubuntu1):

Tim,
  I've verified broken in both 3.16.0-031600rc7-generic and 
3.16.0-031600-generic

Additionally, I noticed that 'dmesg' on both of those showed:

  [1.924049] [drm:i915_gem_init] *ERROR* Failed to initialize GPU,
declaring it wedged

I do not see that in my functional 3.16.0-6-generic .  Granted, that is
based on my very non-scientific 1 boot sample.

Chris seems to have correctly identified the issue.
I tried reverting the xserver-xorg-video-intel to previous versions, but it had 
no affect.

I rebooted into an older kernel and i can still use compiz/unity.

 3.16.0-6-generic : works
 3.16.0-7-generic : fails
 3.16.0-8-generic: fails

I'm seeing in dmesg logs:

stack, including userspace.

against DRI -> DRM/Intel

component if it's not a kernel issue.
[15869.816027] [drm] The gpu crash dump is required to analyze gpu hangs, so 
please always attach it.

This started sometime in the last couple weeks on trusty.

It is moderately annoying in that apport kicks in and walks me through a
collection process and then (I think) *it* dies, so I was unable to
actually report the bug.

Instead, I reported this bug with:

One thing that might be interesting is that /sys/class/drm/card0/error
is an empty file. (and so is /sys/kernel/debug/dri/0/i915_error_state)

just hung again on 3.9.0-2-generic (maybe 2 minutes of uptime).
This time, the mouse cursor moves, but focus doesn't change, and no typing 
input.
screen will dim, then mouse movement brings it back to full brightness.

nothing in dmesg well after 120 seconds waiting.

These files were after dmesg instructed there might be more info in 
'i915_error_state' with message like:

/debug/dri/0/i915_error_state

So, I copied both of the following here.
  sudo cp /sys/kernel/debug/dri/0/i915_error_state 0.i915_error_state
  sudo cp /sys/kernel/debug/dri/64/i915_error_state 64.i915_error_state

Then also collected dmesg as it had more info now than it did when 'ubuntu-bug'
collected it.

Just some information.  Yesterday I saw that 
xserver-xorg-video-intel at 2:2.21.3-0ubuntu1
was available, so I got it, and rebooted.  I figured I'd try with latest kernel 
and X.

This morning in about 2 hours of general use, while scrolling (in ccsm), I hit 
this bug again while running kernel:

  For reproduce, I'm sorry that I can't really help much. It just happens in 
general use. Often times its when I'm using firefox and scrolling.  My browser 
window is always on the second display (non-laptop display) and scrolling seems 
to likely trigger it.
   Sorry for being very vague.
  I can definitely try a ppa build if you or timo could get me one.
  I'm currently running a backlevel version of xserver-xorg-video-intel 
2.21.0-0ubuntu1 as  2.21.2-0ubuntu1 was unusable. (comment 42 on bug 1124576), 
but I'm willing to try a build.

  This is my primary system, so poking around and trying to recreate
bugs or test new X (rather than using something stable-ish) means
completely disrupting my other work.  I'm clearly interested in it all
"just working" for 13.04, so I'm interested in helping, I'm just saying
it is invasive.

To remedy the above (comment 42), I did:
 * install the 2.21.0-0ubuntu1 xserver-xorg-video-intel from [1]
 * update-initramfs -u -k 3.5.0-21-generic
 * reboot
 * I dont need 'nomodeset', laptop LCD and external LCD function fine... 
generally functional X.

I saw this bug again using:

I'm in a much worse state now, though, as booting into the 3.5.0-21-generic 
kernel that used to allow me to actually use my system now
 * wont boot (caps lock key flashes) by default
 * boots, but doesn't see second monitor if I use 'nomodeset' kernel cmdlinle 
parameter, and X has issues (doesn't re-pait under where the autohided panel 
is).

I'm sure there are now some other bugs here.  This is quite possibly the
first time I've rebooted and/or restated X since 2012-02-11 when I apt's
log says: xserver-xorg-video-intel:amd64 (2.21.0-0ubuntu1,
2.21.2-0ubuntu1).

You're right.  Thank you for noticing the error in my report yesterday.
to my knowledge, I've never seen this issue with the quantal kernel.
I've only had the problem with 3.5.X.  Yesterday when I rebooted, and
saw a 3.8.0 and a 3.7.X I didn't think hard and assumed that 3.7 was
quantal, forgetting that raring had a 3.7.

Immediately after it hangs:
 * $ sudo cat /sys/kernel/debug/dri/0/i915_error_state
no error state collected
 * 'DISPLAY=:0.0 xrandr' seems to work.
   'DISPLAY=:0.0 xrandr --output LVDS1 --auto --output HDMI2 --off' turned off 
the monitor, but then never returned.
 * 'DISPLAY=:0.0 gnome-terminal' just hangs
 * when screen dims due to idle (which i've set to 2 minutes), keyboard input 
will brighten the screen again.


then I tried 'sudo stop lightdm', and slept 120 seconds.
Still, no error state in 'i915_error_state'

Then, i tried 'chvt 1', and gave it 120 seconds to return.
at this point 'dmesg' shows the trace, but /i915_error_state still says "no 
error state collected"

If I boot into the quantal kernel, I do not see this issue.  So thats what I'm 
doing as a work around for now.
Each time I forget, and reboot into raring, I hang usually within 15 minutes of 
usage.

It seems that this is likely triggered by desktop switching but thats
not really scientific, just feeling.
