 Results in screen corruption in small or large areas of the form which make it 
unreadable or unusable.
Notified them of trouble and they said " update to Ubuntu version of Adobe 
flash player"
 done: result, no change.
This same problem occured last year at this time with PBS survey form
 Not sure if this is Nvidia problem or Adobe.
Further info:
 Just did a splash of updates that were probably relased yesterday.
using Ubuntu 13.10 on Dell  Inspiron 9300.
Problem is reproduceable and occurred again after new adobe flash player 
install
