We're closing this bug since it is has been some time with no response from the 
original reporter.  However, if the issue still exists please feel free to 
reopen with the requested information.  Also, if you could, please test against 
the latest development version of Ubuntu, since this confirms the bug is one we 
may be able to pass upstream for help.

Thank you for taking the time to report this bug and helping to make
Ubuntu better. Please execute the following command in a terminal after
reproducing the issue, and it it will automatically gather debugging
information needed for this bug:

  apport-collect 777279

Looking at your log, the issue is that the EDID from your VGA is not
being found.  Given that the two monitors are identical, could something
be wrong in your cabling?  Do you use an adapter?  See if swapping out
the cable and/or adapters helps.  If it's just hardware troubles, that
could explain the problems you  had previously as well.

Possibly your second head is not reporting correct EDID information.  I
had a similar issue running through a DP-DVI adapter, discussed here:
