In fact, I have same behaviour (no output) if I replace displayHidden by displayView or displayEdit ... Update: After switching to 3.2 (even clean 3.3 M2 refused to launch) PDF is now generated despite several similar headers present in included pages. But only first TOC brings me to correct sections, others doesn't work in the page itself and navigate to the same sections as first one in generated PDF. Bug? Yes, looks like a bug.
