We purchased the DVD from California Newsreel in 2005. Their website indicates that their distribution is "discontinued". They refer to PBS and Auburn University. CA Newsreel may also have contact info for the rights holder. http://newsreel.org/video/THIS-FAR-BY-FAITH

(A book by the same name is available, but no DVDs.) The production company (Blackside) link appears to be defunct, too, although I am waiting for a response to a query I've send to a different Blackisde Productions. In the meanwhile, I thought I'd check to see if anyone might know of alternate distributors for this title, or other suggestions for films about Black American Muslims.
