Yes. Change the password for your Apple ID and if you've used the old
password on other services change it on them also. NB: from a security
point of view it would be best if you did this from your iPhone. Next,
assuming you use a Windows computer, run scans using your anti virus &
anti malware software to try and determin if the computer is infected
or not. If so, clean the infections or pay someone to do it for you.
Once done, change the passwords for all services that have been used
on said computer. When something like this happens and you simply
can't be sure of how it did, it's always best to assume the worst IMO.

No; I did none of these things. I don't even have a passcode set up on my 
device--an iPhone 4S--although I plan to do so when I get my new iPhone 6 at 
some point. Do you think I should change my password? Of course, I wouldn't do 
it through that email. :)

Emails they say sending all headers and stuff gives them valuable information!

It's important to remember that the from address shouldn't be used to
confirm the validity of a message. If you receive a message from
apple716...@gmail.com you can be certain that it's not from Apple, but
it's very easy to fake an @apple.com address so please don't assume
that a message is genuine if it supposedly comes from apple.com.

Did you change your email address or your security number or enter your 
password for your iPhone several times and lock it up because if you do that 
and hurt so many times and it's not right they will send you a notice such as 
this but I would call Apple on her helpdesk and ask them if they if that's

No, this is not fishy because it states your name, and it states an action that 
you made with your Apple ID. Andy Apple logos sounds legit to me and so does 
the email address.

Hi Christopher, you raise a good question. I have not received one of these 
scans so I'd never have called and asked. But you might want give Apple a call 
and see if they have an address and then posted for everybody if possible.

If you check the from address, you will be able to know beyond a shadow of a 
doubt that it is a scam.
I just got one claiming to be from Apple Lost and Found. The from address was:
iPhonelostandfound@hgb084.cooltrackerspick.rocks
That is clearly an attempt to suck someone into something that will no doubt 
end badly.

Hi, I don’t think there would be any reason for Apple to verify your on line 
account information. I am thinking this is spam. If you’re really unsure, 
contact apple and ask them. I would certainly do this before clicking on the 
update. I have recently gotten a message from what was supposed to look like 
Apple telling me that my iTunes account had been disaccociated from my account. 
You may have noticed the fact that disassociated was misspelled. I spelled it 
just like it was spelled in the message I got. So, as long as spammers can’t 
spell, we will know who they are. And even when they do spell correctly, I 
never click on that kind of link without checking with the source first.
