Fortunately you don’t always need to have your eyes open. The easiest way to do it is to put your screen up to your nose and slowly move it out from your face. Hi, I can use Face ID with this feature on, but it works way more consistently when it's off, particularly if I'm in bright light, such as being outside on a sunny day. So I have it turned off most of the time. If I were in a situation where I was more concerned than usual about security, I would turn it on, though, since if it's on, you have to be looking directly at the phone to unlock it. How well it works depends on how well you can simulate looking at the phone. If you have some vision, I would think having it on would make sense. If not, your eye condition may affect how well it works. By the way, the feature is turned off automatically if you set up Face ID with VoiceOver running. You can turn it on at any time, though.
