Am I experiencing something specific to me, or is this common? I'm honestly tired enough of the issues that I'll go back to the poorly implemented accessibility of Spotify only because they do such a great job on so many things. I turn the Vibrate button on and off. That usually resets the volume issue. I have had occasional volume issues with Apple Music, but have determined, in my case at least, that it is an audio ducking issue. From your description, this may not be the case for you. The next time it happens, try turning off voice over with triple click home, and see if the volume returns to normal. If not, I don't know what is happening.
