Glad you got this straightened out. My wife had the same issue. In her case, 
the headphone jack was clean as a whistle.  The phone was holding on to a 
Bluetooth headset and wouldn’t let go. Apple accessibility gave it the old 
school try and could not figure it out. Time  turned out to be the cure

I know I posted earlier what I thought was a fix, but it went south on me 
again. But I have fixed it now. And OMG, I can't believe it. But it was 
dust and gunk in the headphone jack, a whole lot of it!
I got the gunk out using a toothpick and very very gentle scraping action. 
I can't believe the large piece of dust that came out of my poor headphone 
jack. Really gross!

But now Voiceover is chattering away again. Thanks for the suggestion.

I too am having trouble with voice over. It is sort of hit and miss and I can 
usually get it back but it isn’t very reliable. I am trying to get a new phone 
but that takes a little time. I wonder if there is a problem with voiceover in 
one of the new updates.

Hi ann,

Your headphone jack may have a piece of dust blocking it or something of that 
sort. Try plugging in and disconnecting your headphones a few times to see if 
that helps.

I don't have that phrasing under VoiceOver, Audio in settings if no headphones 
are connected.
Under VoiceOver, Audio I have:
Sounds and Haptics, 
Audio Ducking, which is off.
Auto Select Speaker in Call, on.
Send to HDMI, off.
That's it.
If I plug in headphones, then I have, Sound channels.

So, either your earphone jack has an issue and makes the phone think it has 
headphones connected, or the Bluetooth thinks there are headphones connected.

This is the kind of thing that I would call Apple Accessibility and ask them to 
do a share screen and run the diagnostics they can do remotely.
