Not edited my iPhone five have any problem coming by default up with voiceover.

Keep in mind that when you think that a device is "just out of the box" it
is not necessarily true.  Often it is just out of the box for you but it has
been just out of the box for someone else before you  and maybe just out of
the box for someone else before that.

It truly is hit or miss.  I tried to turn voiceover on before I left the store, 
triple click home was not set. I had to ask the person in the store to turn it 
on and set the triple click home feature.  That was with the iPhone five, but 
with the 4S, I was able to triple click home in the store, and voiceover came 
on.

It must be for some, and not for others.  I got both my 4s and 5 in the mail, 
and had to have my wife turn voice over on.  Upon taking it out of the box, I 
tried triple click home, and got nothing.

Hi,

no. This was introduced in IOS 5 last year.  The reason you had to go into 
settings and set it up yourself is because, your device was set up at the 
store.  You must use VO at the setup process for tripple press home to be on by 
default.
