Hi Chuck,

Go to VoiceOver Settings. In Speech, make sure no languages are listed under 
the Rotor Languages heading. If any are listed, delete them by touching the 
language name, flicking up or down to Delete, and double-tapping.

Now go to Braille Settings, and double-tap Braille Tables. Make sure that only 
a System braille table is listed. If any others are there, delete them. These 
rotor items should disappear.

If that doesn't help, restart your device.
